Gurgaon, 23.11.2018: A team of doctors of Paras hospitals, Gurgaon has successfully performed a complex surgical process involving simultaneous liver resection and reconstruction of patient suffering from hilar cholangiocarcinoma, a tumour that affects the biliary ducts of the liver.
Patient(name, age) is from a very poor background and he is a school teacher from Palwal district, who is vegetarian, teetotaller and non-smoker. He visited a local hospital in Palwal and then to other hospitals in Faridabad with the symptoms of abdominal pain, or discomfort in abdomen, weight loss, anorexia, fatigue, intensely itching skin, yellowing of the eyes but he was given the treatment of Jaundice.
With no improvement in the condition, he was referred to Paras Hospitals, Gurgaon, were he consulted Dr. Vinay Samuel Gaikwad, Associate Director & Head, Surgical Oncology. He wasadvised to undergo few tests which includes imaging tests like PET CT, Liver MRI,blood tests with high level tumor markers. With these diagnosis, there was a significant finding that the patient has Hilar Cholangiocarcioma which is very rare tumor. The exact prevalence is unknown, but accounts for less than 2% of malignancies. Patient and the family members were counselled about the tumor and surgery consequences. Family took the decision for the surgery.
Surgery took place for 4-5 hours with challenges as it was equal to performing two surgeries at one go – on one hand, the more than half of the liver that was affected by the tumour had to be removed while preserving enough liver for normal body functions; on the other hand, the entire biliary system had to be reconstructed to enable the smooth flow of bile.

Hilar cholangiocarcinoma is a rare tumour of the biliary ducts of the liver. The diagnosis of the disease is often delayed and are rarely detected before they have spread to other parts of the body. The diagnosis is delayed mainly due to the late appearance of the symptoms, which is usually jaundice. Treatment of hilar cholangiocarcinoma include surgery, liver transplant and chemotherapy.
